Getting Started
Whether you will be here as an inpatient or outpatient, you will need to share basic information with hospital staff. This process is called registration.

You may now choose to pre-register for many services by completing a secure online form. This will save you time once you arrive, and in some cases, allow you to report directly to the area that will provide your care.

Many patients will register once arriving at the hospital. This means they will report to the main registration area. Registration is located in the west end of the Main Lobby behind the Information Desk. Once you enter the Registration area, a friendly clerk will guide you through the process and assist you in filling out any necessary paperwork.

Before arriving, remember to bring with you all paperwork given to you by your physician, along with your insurance cards. Other patient information that will be asked during the registration process includes:

• Full name and address
• Date of Birth
• Social Security number
• Name of an emergency contact
• Insurance information. We will need to scan your insurance cards into the registration system. 
• Name, address and birth date of policy holder, if different from patient.
• Name of employer
• Name and address of where to send the bill if the procedure is for liability or workers' compensation

We understand that a hospital can sometimes be a large and intimidating place. If you need help with directions at any time, please ask a staff member or volunteer, or inquire at the Information Desk. If you have questions or concerns about the registration process, please call 812.353.5166.
