Diversified Investment Advisors has been selected by Bloomington Hospital to administer the Bloomington Hospital, Inc. 403(b) Retirement Plan for hospital employees. You may also need a Beneficiary Designation Form.
- Log in to the secure site to see your current account balance, contribution percentage and fund selections (only your social security number is required to set up your account).
- Diversified provides planning tools, checklists, videos and more online. This is the place to go to plan your future.
- Make all changes directly with Diversified. Human Resources will be notified if you choose to change your contribution percentage and will make the change in the payroll system.
- Diversified representatives come to Bloomington Hospital for one-on-one and group educational sessions three to four times each year.
- Quarterly statements are mailed to all plan members, or you can request they be made available to you online at the Diversified website. In doing so, you would receive them much sooner than in the US Mail.
- Bloomington Hospital Human Resources staff members are unable to provide retirement advice or direction because of federal law.
- Do you have funds in another plan? Qualified roll-overs are allowed. Contact Human Resources for more information about how to roll-over into the Hospital’s 403(b) plan.
